I disagree with the Elements of Style on this and I always have. The punctuation should go inside the quotes only if it was part of the original phrase being quoted.
My name is Michael but everybody calls me "Mike".
That makes logical sense. The nickname is in double quotes.

The nickname is "Mike-period"? Would you write
"Mike." is my nickname?
That period does not belong inside the double-quotes.
As for emphasis, what exactly does an italicized "." sound like? If you want it to be emphasized, make it an exclamation point or interrobang.
The only time I include the punctuation as emphasis is in headers which, in Markdown, are thankfully line based.
I'm sorry if this sounds confrontational. I have very strong opinions about punctuation and quotation marks. I didn't mean for it to sound so angry, just passionate.
